Subject: Monogenic Diabetes Registry Participant-Portal

Description: We are creating a participant-portal for our Monogenic Diabetes Registry participants. Participants can log in to a webpage where they will be able to complete study tasks and upload documents.

We would like an email to be sent to notify research staff when participants upload a document. We are working with CRI to program this feature, but need an email address specifically for this purpose.
