Subject: Cellular Service Change

Description: We would like to establish a list host email address to send email communication to cellular customers on our University of Chicago ATT account. We are changing carriers to ATT First Net and will need to send out a new sim card (for cell service activation) to all of our ATT cell customers therefore we will need to ask for their shipping address & ATTN name.

In the email communication to our customers we will provide a link to a google form to collect addresses and additional needed information.

We would like for the list host to receive replies but we expect the majority of return communication through the google form.
